We've had very different experiences, in my >500 hours of gameplay I've only seen a few cases of racism (all of which were voted out) and one case of transphobia (who got torn to shreds in chat). I guess I've just been a lot luckier than you. Bystander syndrome is a thing, what you see as apathy might just be people waiting on someone else to take the first step to removing the problem. (that said, depends heavily per lobby. not gonna pretend there wont be cases where the votes failed/you get counter voted, but in that case you wouldn't want to be there anyways right?)
